Isolated ST depression in aVL, without ST elevation in the inferior leads, may occur in a significant percentage of patients with inferior wall infarction.

Electrocardiographic Criteria

  • Transient ST-segment depression in aVL (reciprocal change), with or without subtle ST elevation in the inferior leads

ECG in ST Depression in aVL

Electrocardiogram with ST depression in aVL

ST depression in aVL. In a clinical picture consistent with acute coronary syndrome, ST depression in aVL with a negative T wave, with or without subtle ST elevation (< 1 mm) in the inferior wall, suggests acute coronary occlusion of the right coronary artery.
